- Upon inspection, found serpentine belt tensioner had come off. Pulled off alternator and the mounting bracket for the tensioner and alternator and found that the bolt for the tensioner was too short and did not thread in more than 2-3 threads. We cleaned up the threads and installed a longer bolt to correctly secure the tensioner to the engine/bracket. We also replaced the tensioner due to it making excessive noise when running. Head gasket test was normal, and no further issues have been found.
